CAMPAIGN CONSENT FORM

PERSONAL INFORMATION

Lavender Mobile Spa services the following areas: 

Aubrey, Allen, Addison, Carrollton, Dallas, Garland, Denton, Frisco, Fort Worth, Grapevine, Grand Prairie, Little Elm, Irving, Plano, Prosper, Richardson, McKinney, Arlington

Do you commit to the following deliverables?

  • 1 in-feed post 

  • 1 story post on Instagram with your unique LMS link and code with @lavendermobilespa tagged

  • Submission of content's performance insights 1 week after your content goes live

Do you understand that this is a gifted collaboration with the following services to be accommodated for your participation?

  • 30-minute Facial for yourself and 3 guests

  • 30-minute Swedish Massage for yourself and 3 guests 

(package valued at $580)

Do you understand and agree to comply with the campaign timeline?

  • Schedule your spa party with to take place anytime between Jan 25 - Feb 18

  • Content must be submitted for review: Feb 27

  • Approved content Posted: March 1-March 5 (excluding weekends)

Influencer Collaboration Agreement

Content Ownership and Usage Rights:

The influencer retains full ownership of the content created for the Lavender Mobile Spa campaign. Lavender Mobile Spa is granted the right to repost the influencer's content on their social media platforms. However, Lavender Mobile Spa shall not use the content for paid advertising on any platform.

Cancellation Policy:

In the event that the influencer wishes to cancel their participation in the campaign, they must inform Exalt Management by January 23, 2024. Failure to provide notice by this date may result in the influencer being ineligible for future collaborations with Exalt Management and its clients.

Exclusivity and Non-Compete:

There is no exclusivity or non-compete clause. The influencer is free to engage in promotional activities for other brands, including competitors, before, during, and after the campaign period.

FTC Compliance:

The influencer agrees to adhere to all Federal Trade Commission (FTC) guidelines regarding endorsements and disclosures. This includes clearly disclosing the partnership with Lavender Mobile Spa in all campaign-related posts.

Content Standards and Brand Alignment:

The influencer agrees to follow the content brief provided by Exalt Management, ensuring that the tone, style, and messaging align with Lavender Mobile Spa’s brand image.The influencer shall avoid any content that could be deemed offensive or detrimental to the Lavender Mobile Spa brand.

Post-Partnership Evaluation:

Exalt Management will conduct a post-campaign evaluation to assess the effectiveness of the influencer's performance and the overall impact of the campaign.

​

Confidentiality:

The influencer agrees to maintain the confidentiality of any sensitive information shared by Exalt Management or Lavender Mobile Spa during the course of the campaign.

​

Dispute Resolution:

Any disputes arising from this agreement shall be resolved through mediation. If a resolution is not reached, legal proceedings may be considered as a last resort.
